Transfer Students from other than Hawaii Public Schools
     When registering, please present:
         .  TB Clearance issued in the U.S.A. (current within the year)
         .  Physical Exam (current within the year)
         .  Birth Certificate
         .  Record of Immunizations
         .  Release records from former school

Transfer Students from Hawaii Public Schools
     When registering, please present:
         Release Records from former school

EXIT PROCEDURE

Call the registrar at 573-8745 to request Release Forms.  If possible, give at least one week's notice.  Please make sure all obligations are paid and all books are returned. Information needed:
         .  Last day at Kalama
         .  Name and address (if out of state) of new school
